Our Vaccine Schedule:

2 months: Pediarix (DTaP, Heb B, Polio) HiB, Prevnar, rota oral drops

4 months: Pediarix (DTaP, Heb B, Polio) HiB, Prevnar, rota oral drops

6 months: Pediarix (DTaP, Heb B, Polio) HiB, Prevnar

9 months: No vaccine

12 months: Proquad (MMR+Varicella) HiB, Hep A. Hemoglobin & lead check

15 months: DTaP & Prevnar

18 months: Hep A#2 vaccine

2 year: Hemoglobin & lead check, school requirement

3 year: Blood pressure yearly, weight and development screen yearly

4 year: Proquad (MMR+Varicella) Kinrix (DTaP + Polio) Booster vaccines

11 year: TdaP (tetanus shot) Meningitis ACWY vaccine (menveo), Cholesterol check, HPV (Gardasil)

16 year: Meningitis ACYW & Meningitis B vaccines

Ours is a strict Pro-vaccine Practice and in order to be fair for all our vaccinated children we do no accept patients who don’t believe in vaccinating their infants and children. (Our Vaccination Policy)
